- Review all medications including over the counter and natural health products and identify any that may no longer be necessary or beneficial.
- Discuss our findings with you and collaboratively develop a plan to reduce medications where necessary or optimize your current regimen.
- Work with your care providers to implement new plan, where necessary, in order to safely and gradually reduce or stop medications.
- Monitor your progress and make adjustments to your plan as needed.

- Information gathering: We will spend some time collaboratively gathering information on all your medical history, medications, regimen, side effects and your goals.
- Comprehensive Medication Review: We'll conduct a thorough review of the information gathered over a period of time depending on the complexity of your case.
- Personalized De-prescribing Plan: In collaboration with you and your care provider, we'll develop a personalized plan to optimize your medication regimen.
- Ongoing Monitoring & Support: We'll monitor your progress, address any questions or concerns, and make adjustments to your plan as needed.
